Apply Button Needs to Pre-Populate 2 Form Fields On Different Page

Hey guys. Newbie here (first post). I’m working on a page with a repeater. Each listing has an ‘Apply’ button (circled in blue). When a user clicks that ‘Apply’ button, I’d like to pre-populate 2 fields from this listing (circled in red) on a Wix form located on another page. Any idea how to get this done?
