Help in creating more complex forms?

I am attempting to create a field in wix forms that behaves in a similar way to tick boxes. I have 50 criteria that i want a user to be able to choose 10 of those 50 criteria and form a number rating (1-5) against each of those ten criteria in order of highest priority. I can’t seem to find a form field that allows me to do this. Does this actually exist? And if so, where do i find it?

I’m on a business plan using Wix Editor

I’m trying to get something similar to the following:

criteria A [4]
criteria B [ ] (intentionally blank)
Criteria C [3]
Criteria D [5]

etc.

Any help appreciated

Would it be an option to have 10 dropdown fields (each with 50 criteria) and 10 rating inputs in a two column layout?

Thanks - this is a good idea, however it doesn’t answer what i’m looking for as it restricts the inputs I’m trying to capture
