How do i add this feature?

I am making a website for a rental company. The two pictures above are how our website looks currently! The picture below is of a website that our client liked and hope we could do similar features on. My questions is how would I be able to code a feature like the one below, where the customers, after selecting the quantity of chairs needed, have a choice to select the date and like they want to reserve and leave their phone numbers emails. The owner then gets an email and responds to the clients through email.
