How to auto populate form fields

I just want to auto populate the form fields like if any user have apply for the job then admin should notified that ‘x’ user has apply for ‘x’ job with relevant input fields.

In screenshot I want to auto populate the Job ID and it could be Job title too.

Just to show a SCREENSHOT and then waiting for a READY2GO-SOLUTION will not really work.

  1. Where is a screenshot of your already prepared and existing DATABASE ?
  2. Where we can see the first steps, showing us that you already have tried to code it by your own? Where is your code?

Normaly this is not a place where to get free CODE-SOLUTIONS for your project!
