How to know what page a form was sent on

I have a dynamic item page that has a form on it.

The page titles are 1) Dogs and 2) Cats

The dynamic link endings are /dogs and /cats.

When a person fills out a form I want to know which dynamic page they were on. Is there either a way to either:

  1. grab the title from the dynamic page and autofill it inside the form
    or
  2. grab the link they are on and send it with the form submission?

I just need to know what page they are on so if there is a better way let me know! Thanks for any help!

@decadthelena This was actually updated a while back and now you can see which page the form was submitted on.
