It seems to me that you’re on the right track, but you’re probably just missing a Submit button. See the section Create a Submit Button of the Tutorial: Creating a Form to Store User Input in a Database Collection . You can view the video and read the full tutorial to get the “real skinny” on creating a form to upload data to the database.
