Hi,
I am trying to create a referral form for a dentist web site. As part of this form, the referring person would have to select between 32 adult teeth and 20 baby teeth when they are choosing the TOOTH. I am having difficulty in how to create a form where multiple radio buttons will feed into one database field and 2)how to allow the user to select multiple options at the same time. I have attached an example of this form from other websites showing the different options for teeth numbers
