Stuck with form

Hi all, I’m trying to replicate a form for our Wix site but I don’t think it’s something that’s available on the normal Wix Editor?

It needs to be more or less this, same wording as its for the same organisation but they’ve used Wordpress and a Gravity Form i think.

Can anyone advise on how I can get this sort of thing where a button or more options appear depending on what answers are input by the user? Are there any resolutions where I could amend it in the future without too much coding knowledge? Have these types of forms got a proper name?

It seems that what you’re looking for is a Multistage Form . The example splits a form into multiple stages using a slideshow.

Thank you. This is too much for me to figure out - I’m going to need to hire someone who knows what they’re doing. How do I hire someone that will definitely be capable of this?
