I have the categories displaying correctly (connected to a category dataset)
I need the boolean value to connect to the category dataset
I need the user input (boolean checked) to be connected to the form submission database.
Any ideas?
(Please check steps 1-5 at the top of the page for user flow/process. Im stuck at the boolean value and form submission phase)
Any advice or support is well appreciated. I even tried outsourcing this but nobody I talked to had the solution.

All you had to do was, if you already had a “request” collections”, create a column of Boolean type. Then connect your checkbox to database.
