Creating a User Input Form with Dynamic Pages?

Hi Guys,

I’m trying to create a form on my Wix site, but across/splitting it up onto multiple pages. Basically, on the first page it asks the visitor to put in their name, email, and phone number, but then I want them to click on a “next” button to take them to a new page to answer additional questions (their budget, their style choices, etc) I want all of their input to be captured on the same line in the collection of the database. How can I do this? Do I need to create dynamic pages and link them using a “next” button?

Thank you Sam! Is there a way to show a certain slide based on the type of input a user gives? For example, if they select a radio button (Choice A) on one slide, can the slide show be set to show a different slide than if they chose Choice B?
