custom registration forum code issue

I’m trying to create a custom registration forum. I need fields other than email and password. I followed the tutorial on how to make a custom registration forum. The forum is in a lightbox and a dataset is connected to the fields. When I test it my information goes into the dataset but I’m not getting a member request. It’s saying that there is an error in my script but I’m not seeing it.
