Customer feedback options

“I’d like to create a light box where there my customers would have the option to vote between two buttons if a particular page was helpful or not.I’d like to collect the votes as feedback from our users, and I’d like the options to take them to different screens based on that selection.
If they find it helpful, we’d like to give them the chance to subscribe to our newsletter and be added to contact group for site updates. If they select that it is not helpful, I’d like to link them to a contact form where they can go into detail about what was not helpful for that page.
