Dataset selection transfer into a submission enquiry form problem

I have a page on my site that uses dropdown boxes to filter a dataset that has been creted to show services in someones area/by day/experience level etc.

The bit I am struggling with is I want to then use the selected information to form an enquiry that the customer can submit without having to enter those details again? Is this possible as it has nearly beaten me!?
