Designer notes

As I’m designing my site I make design choices that maybe I’ll forget in a few months, or that other designers working on my site should know about. I would love to be able to record a designer note for each page as I develop and maintain it. The note would of course never be visible to the end user, and should be “hideable” if the developer so chooses. Ideally it captures my name and date of entry but I would be OK with a simple text box. Same comment for databases. Some fields require a little bit of explanation.
