Dynamic pages Individual Forms

I have an issue. I have clinic website. There are over 60 health problems page and in each page there are forms. Once a person fills the form from a specific page, I can see which page they filled the form because of the form name. If they visit "Back Pain’ page, form in the “Back Pain” page named as it is. And also each form has its own automations email. If they fill the form “Back Pain”, they recieve specific automation about back pain.

Now I wish to change these pages into Dynamic Page but I couldnt figure out how to do the same with forms as it was before. When I add a form to a dynamic page, same form appears in all dynamic pages so i will not understand from which health problem page that form is filled.

Is there any way to solve this and specify each form for each page?

If i understood your setup right… you have the following situation…

  1. You have a bunch of sites → [Site-A, Site-B, Site-C, Site-D…] and on every of your sites you have placed a different unique form to collect USER-DATA.

  2. Surely you will have ONE DATABASE, where you will store all that collected data.

  3. Now, you want to convert your project into a → DYNAMIC one.

To do it dynamic, perhaps you want to use a “Multi-State-Box”, where you can put all your forms inside. Every state of the MSB would have a unique form inside.

Now you would be able to cennect dynamicaly every single STATE of the MSB with every wished form you want.

You will need the index-number, which will represent the number of choosen STATE in the MSB.

Thank you so much for your reply. I tried your way and I see this is the way but finally I found myself to deal with Codes which I escape cause I dont know how to do codes.

While I was struggling I found a way. My static page limit is almost will be over but there is no limit on creating dynamic collection, i guess. So I will create as many collections as i need. I figure out each collection can be design as a static page. I tested and it seems working. I dont know If I will have any complication after.

I add Dynamic page. I choose ‘Custom’. I choose then ‘Customised’. There I dont choose ‘Dynamic List Page’ but only ‘Dynamic Item Page’.

Like this method, I will have many independent pages like ‘Static Page’.

Hope this will help me without struggling with Codes.
