fails provision request Error when trying to add external MySQL Database

I am trying to connect to an external MySQL Database (on Google Cloud) using this adapter (https://github.com/wix/corvid-external-db-mysql-adapter). I am getting an error

“Unable to fetch result (Connector with namespace ‘wixtest’ and instanceId ‘b6fa1942-76ef-4b08-b19d-9adc66ad4341’ fails provision request)”

Does anyone know how to fix this error? Thanks.
