Skipping the checkbox on submission form sends correct triggered email. Checking the box omits the contacts name in sent triggered email.

I created a lightbox with a submission form:


When a name and email are entered and the checkbox is skipped before clicking ‘Give Me That Guide’ the triggered email sends properly the contact name (Disregard the differences in contact name like John or Jopa or email address. For illustration purposes only).


However, when the checkbox checked before clicking ‘Give Me That Guide’ the triggered email sends without the contact name (in this case Jopa). (Remember, disregard the differences in contact name John or Jopa, or email address. For illustration purposes only):


Also, skipping the checkbox before clicking ‘Give Me That Guide’ does not send name and email to dataset.
Clicking the checkbox before clicking ‘Give Me That Guide’ does not send name and email to dataset.

This is driving me near crazy. Suggestions?
