Upload Button in Wix Bookings Form

Is it possible to add an upload button on the automatically generated Wix Bookings Form when customers are filling in their information to book an appointment?

I tried to add it using the User Input Upload button and it works, but it shows up on the Service Page, Service List, Appointment Calendar, Add Your Info (this is the only page I want it to show up in), and the Confirmation page.

Currently there’s no integration of Wix Code with Wix Booking. However, it’s on our backlog. You can suggest it as a feature request here .
