Add custom fields to donations form created by Wix Donations App

Question:
I would like to add custom fields to donations form created by Wix Donations App. I am wondering if there’s a way to do so via Velo?

What are you trying to achieve:
I would like to add additional fields for user to enter when donating.

What have you already tried:
I tried old forms with Donation option, which lets me add custom fields, but there I can either have donation amounts or custom amount, while I would like both, like Wix Donation App does.

If you’re already familiar with Velo, then you can easily create a custom donation widget with as many fields as you like.
