add text automatically in a field when a document is uploade

how can I automatically add a text in a field when I upload a document, and if I do not add text, say empty document, help me

Read the Tutorial: Creating a Form to Store User Input in a Database Collection for detailed instructions how to upload an image. Then just change the image in the tutorial to a document .
