Automatically generate the input field inside a dynamic page when the page loaded

Hi guys, i’m making a checkout passage under my item description (check the picture attached), know that the page is a dynamic page for certain product, i want to make the information below to match the information up which is coming from the production collection, so the customer after he pressed the button down the information matching the dynamic infos will save in the order collection. thanks in advance guys
