Create form that populates google maps

Hi there. I am accustomed to adding Google maps to my sites, and adding different locations, etc - always been very simple and clean.

I am trying to create a form that guests can enter a Start address and Destination address, a date and a time, that will open the Google maps site with their information already populated. Is this possible in Wix? I have had no success… Hoping to create something like the below from a previous Wordpress site our company had.
