Hello! We are a health care non-profit with a “Take our pledge” page where supporters can sign on. We would like the names of our supporters to be displayed underneath the form so that other viewers can see them.
The form has the fields:
- Name
- Credentials
- Email
- State (drop down)
- “How to you self-identify” (free text)
Ideally, we would like the signatures to be grouped by state, to facilitate organizing on a local level.
Any tips on how to get this to auto-populate?

Hi, I’m hopping on this discussion two years later! I’m looking for the same thing – I’d like a petition on my website that people can sign. Then, I’d like their names to appear below the petition, so others can see who’s signed before them.
The auto-population point is that @universalhc doesn’t want to have to manually update their website with the new petition signers; they want someone to sign and their name to automatically go beneath the petition.
So… any help? @universalhc did you figure out how to do this?
