I need the functionality of the contact form on http://www.daviswilliams.com/ (the “What is Your Risk Number?”) Call to action. This is a contact form with multiple pages that generates an automated, custom response based on the inputs.

If you are still getting stuck, then just use the search function in this Wix Code forum and look up sending data to lightbox, as there are many previous forum posts with the same or similar question.
