I want to ask customers a few simple questions to allow me to present them with the best option for their needs. How can i do this?

I am selling training packages to allow customers to train for events. I would like them to answer 2 multiple choice questions (event types and ability level), and then ask the date of their event. We can then use this information to suggest the best product (training plan) for them to buy to suit their needs.
Has anyone got any idea how this can be achieved?
