Can anyone please help me to fix a Wix form?

What are you trying to achieve:
I would like my form to be submitted, it is a questionnaire for a product selection.
What have you already tried:
I have been trying for 2 days to create the form so that it can be submitted, but I always get the error message that it does not work and that I should try later.
Additional information:
I have like Basic and already a contact form on my site, this works wonderfully, but now I have added a second one and this one does not work at all. The form also contains pictures for the individual products and multiple choice boxes.
