Title of all blog posts is the same on all pages

The title of all the new blog posts is the same “[site name] | Post”, this has to be fixed, it’s an SEO disaster.This leads to a title catastrophy in the search engine result pages. How can I use the old blog, which seems to work fine, this is just unacceptable.
