Would it be possible to:
1 Have different levels of access to one Wix Online Program?
2 Give 15-20 people easy acces to one Wix Online Program if only one pays for it?

For the first option, there isn’t currently a way to have only some people have access to some steps though you can decide the visibility of the programs with different options such as it’s only visible to users you invite.
As for the invitation, you can manually add participants to the program or share a link for to the invite link.

About the second part of it, I think people would still need an account to either add them manually or share a link with them? Or isn’t that necessary?
You’re right that for the invitation, users do need to already members to send the link to the event. You can see an email I don’t have as a user cannot be invited.
If you want to send it to users who don’t have an account, the link would be the best way which would invite users to sign up and become members.
