I’m having trouble with
Wix Old Forms app - some user submissions are not appearing in the form collection/submission table, even though users receive success confirmation after submitting the form. Most submissions work fine, but randomly some users’ data doesn’t save to the database.

What I’m trying to do
I need all form submissions from my scholarship application form to be reliably saved in the Wix form collection so I can track and respond to every applicant. Currently, some users are successfully filling out and submitting the form, but their data is not appearing in the CMS collection or the Forms & Submissions dashboard.
What I’ve tried so far

Verified collection permissions are set to “Form Submissions”

Checked that the form is properly connected to the collection

Confirmed all required fields are mapped correctly

Tested the form myself multiple times (my test submissions work fine)

Reviewed the dataset settings

The form includes these fields: Name, Phone (with country code dropdown), Email, Program selection dropdown (Online MBA), State selection dropdown, Radio buttons for enrollment history, and a message textarea

Form has reCAPTCHA v2 checkbox enabled (currently placed near the middle of the form)

The issue is intermittent - approximately 70-80% of submissions save correctly, but 20-30% are missing despite users reporting they submitted successfully

I’m not receiving email notifications for the missing submissions either, which suggests they’re failing during submission rather than just not displaying

Form has multiple required fields (marked with red asterisks)

Users are filling this form on both desktop and mobile devices
Has anyone experienced similar issues with the old Wix Forms where submissions randomly fail to save? Could this be related to reCAPTCHA token expiration since the form is lengthy, or is there a known bug with the old forms app?
