Enabling a customer to upload a file during Product Check Out

I’d like to enable a customer to upload a file (photo) during the Order process. The file should be captured along with other Order information in Orders collections. How can I do it?

Perhaps some additional information. I am trying to modify a Product page to enable customers to upload a picture file during check out. I found an option to add “Custom Text Field”. Looking for a way to add “Upload” button so the file uploaded by Customer will be captured along with Order ID and other customer information. Please help
