I need to create a better form for my reservations. I need to be able to find out if anyone has food allergies, ages and names of all guests etc. I am currently using the Wix reservation app and do not know how to include a better form within that app. Please advise.

Unfortunately, Wix Code does not have an integration wix Wix Reservation App yet.
you can consult Wix Support about using the app (they know better than us - we’re specialised in Wix Code).
If that’s not helpful, I can suggest an alternative…
You can use wix code and the Editors User Input elements to create whatever form you like.
How about creating it in a hidden page, and put a link to that form on your Reservation app page?
Like… a user makes a reservation and then sees a link that says: “Click me for special requests!”
This will take them to the page with the form… the data will be submitted to a database in your site.
