I am currently creating a form that has the same working process in the image.
In this, there is a multi-question in different pages and the user just the click the section appears on the screen. After click, the information is stored into the database.

No you would have to make something like that up on your page.

Another option for you is to use an online form builder like typeform which does what you ask and use their free option.
Others like Jotform and many more are out there too, just search online!
You will have to sign up with them and create the form, however you will then be able to copy the html code that they supply to you and embed that into a html iframe on your page.
Although as Heath says, you’ll still need to know how to pass data between the html copmonent and your page etc.
