My submit button for my custom form works on my live site as long as I’m not trying to upload a photo. How can I fix this?

What are you trying to achieve:
I want site visitors to be able to fill out a custom form and submit photos and videos of the issue they are having so our techs will know exactly what is going on. It’s a service based website for HVAC.
What have you already tried:
I’ve checked permissions, field types, links and tested numerous times and this is what I’ve figured out that the problem is.
Please help!
