User Inputs Suddenly Disconnected from Data

I have a number of custom forms on my site with user inputs connected to fields in a database. I logged in to test something this morning and the user inputs were inexplicably disconnected from their fields. The submit buttons were also disconnected. The dataset did, however, still show on the page. I haven’t made any unusual updates on the site or changed anything on these forms. What could have happened to make this occur site-wide? Thank you.
