Wix Members Form - Checkbox

Hi! Looking for someone to help with the necessary code for a simple task.

Within the WIX members sign up form I need to add a checkbox with four options. From each of those four options, the user will be added to an email list based on the checkbox chosen.

Let me know if you know how to do this!

I was trying to do this for the community site as well. There is not a way that I know of to link custom signup fields to the email list - but I do know that there are some interesting changes coming that will allow something close.
